How long does it take to get into ketosis on a keto diet?
On a keto diet, blood ketones usually rise above 0.5 mmol/L within 2 to 4 days, the time it takes the liver to empty its sugar store. The body then needs about 3 to 4 weeks to get used to it: this is called “keto-adaptation.”

What fuel does the body use, week after week?
Insulin drops and fat tissue releases its fat. The pancreas sends less insulin and more glucagon. The liver empties its sugar store, then turns part of the fat into ketones for the brain and muscles.
Why does the scale drop so fast in the first week?
Sugar stored in the liver and muscles (glycogen) is stored with water. When the store empties, the water leaves through the kidneys, along with salt.
Our calculation: glycogen lost (liver and muscles) + 3 g of water per gram of glycogen, the low end of the 3 to 4 g range (Kreitzman, 1992). Water lost with salt is not counted here.
Can the brain run on ketones?
The brain burns almost no fat. It runs on sugar, and takes up ketones when there are some in the blood: each sugar cube replaced by a golden drop gives the same energy.
Ketosis or ketoacidosis: what is the difference?
Both words are about the same ketones, but not the same level. Ketones release a little insulin, which brakes fat tissue: the loop regulates itself. Without insulin, the brake fails. Drag along the scale.
Why does LDL rise in some people and not in others?
With little sugar, muscles run on fat. According to one hypothesis, in a lean person a good share arrives from the liver in “trucks,” the VLDL, which become LDL once emptied. Drag the BMI slider.
Frequently asked questions about ketosis
How long does it take to get into ketosis?
With less than 50 g of carbs a day, blood ketones usually rise above 0.5 mmol/L between day 2 and day 4. In one trial, the average crossed this threshold on day 4, by day 2 with MCT oil, and one person never reached it (Harvey, 2018).
Why do you lose weight so fast in the first week?
Mostly water: each gram of glycogen in the liver and muscles is stored with 3 to 4 g of water (Kreitzman, 1992). When it goes, the water goes too: about 1 kg (2.2 lb) in the first week by our calculation.
What is the “keto flu”?
A flu-like feeling, headache, fatigue, nausea, or dizziness at the start of the diet. In a survey of online posts, these complaints peaked in the first week and became rare after 4 weeks (Bostock, 2020).
Are ketosis and ketoacidosis the same thing?
No. On a diet, ketones stay below 8 mmol/L and blood pH stays normal, around 7.4 (Paoli, 2013). Ketoacidosis happens when insulin is lacking, mainly in type 1 diabetes: the blood turns acidic, pH below 7.30, with ketones above 3 mmol/L (Umpierrez, 2024), about 9 on average on arrival at the hospital (Kitabchi, 2009).
Are urine ketone strips reliable?
They measure acetoacetate, one of the ketones (Urbain and Bertz, 2016). Over the weeks, the kidneys take more of it back: the strip can fade while blood ketosis continues (Fante, 2025).

True or false?
In the first week, you mostly lose fat.
False. Most of the weight lost at first is water, gone with the glycogen (Kreitzman, 1992).
In ketosis, the brain no longer uses any sugar at all.
False. Even after 5 to 6 weeks of fasting, ketones cover about 60% of its needs; the rest is sugar (Owen, 1967).
The keto diet was first used to treat epilepsy.
True. Russell Wilder proposed it in 1921 at the Mayo Clinic to reproduce the effects of fasting on seizures (Kossoff and Hartman, 2012).
